【個人情報の取り扱い】

このシステムは、InterQuest社の予約システム「リザエン」を使用しています。個人が予約できる健康診断の種類を振り分けするために、学籍番号、教職員番号、生年月日、性別、所属地区、課程を事前に設定しています。これらの情報は健康診断終了後に適切に削除いたします。

また、別のシステムで回答いただいた問診内容やその他の健診情報とのデータ連携は行っておりません。健診当日の本人確認は、別途学生証・教職員証で行わせていただきます。皆様方のご理解とご協力をお願い申し上げます。

Handling of personal information

This system uses InterQuest's reservation system, “Riza-en.” In order to pre-determine the types of checkups that a student can book, the following information is entered in advance: student/faculty/staff ID number, date of birth, gender, and campus affiliation. Your information will be deleted as appropriate after the completion of the health checkup.

Furthermore, we will not link this data to your responses to questionnaires in separate systems or other health information databases. On the day of the health checkup, we will verify your identity with your student/faculty/staff ID card. We appreciate your understanding and cooperation in these procedures.
